White Elm Wellness Institute & Training Spa cannot assure admission to all applicants nor guarantee availability of classes. The institute reserves the right to determine whether an applicant possesses the qualifications to be successful in a program of study at the institute and in the vocation for which the program provides preparation. There is a non-refundable $100 Application Fee required for submission of an application for admission. The institute requires that each applicant, prior to enrollment, must submit acceptable High School diploma, transcripts, or equivalent from an acceptable accredited high school. Transcripts must show high school completion. Foreign High School Diplomas and Diplomas not in English must to be translated and evaluated by a recognized agency for verification of validity. That outside agency must be qualified to translate the documentation to English and confirm it’s equivalent to a U.S. High School Diploma. Ability to Benefit is not recognized by the school. The institute also accepts General Education Diploma (GED), or other Department of Education recognized high school diploma equivalent such as the HiSet Exam. Proof of home schooling at the secondary level as defined by state law, or proof of an an academic transcript of a student who has successfully completed a bachelor's degree or at least a two-year program that is acceptable for full credit toward a bachelor’s degree. Students must also submit proof of age and be at least 17 years old and must not be enrolled at a primary or secondary institution school. Students enrolling in the Massage Therapy programs must submit proof of age and be at least 18 years of age on scheduled class commencement. White Elm Wellness Institute & Training Spa does not discriminate in its employment, admission, instruction, or graduation policies on the basis of age, race, ethnic origin, creed, religion, color, gender, sexual orientation, financial status, or area of origin (nationality) or residence, nor does the school actively recruit students already enrolled in or attending another institution offering similar programs. Texas law restricts the issuance of occupational licenses based on a license applicant’s criminal history; and authorizes the Texas Department of Licensing and Regulation, in some cases, to consider a person convicted, even though the person was only on probation or community supervision without a conviction. As an applicant/enrollee in an educational or training program that prepares individuals for issuance of an occupational license, Avenue Five is notifying you that if you have been convicted of an offense or placed on probation, you might not be eligible for an occupational license issued by TDLR after program completion. TDLR’s criminal history guidelines are available at www.tdlr.texas.gov/crimconvict.htm and include restrictions/guidelines TDLR uses to determine eligibility for an occupational license. You have the right to request a criminal history evaluation letter from TDLR, which is explained in more detail at www.tdlr.texas.gov/crimhistoryeval.htm. Section 53.152, Occupations Code, requires that notice be provided to each applicant/enrollee regardless of whether the he/she has been convicted of an offense. State law also requires TDLR to order an educational program provider to refund tuition, license application fees, and examination fees if TDLR determines the institution failed to provide notice to applicant/enrollee and license application was denied because of criminal history. Federal financial assistance can not be used to pay for the Application Fee. The Application Fee is non-refundable except in special circumstances as outlined in the schools refund policy and as required by TDLR rules. The school cannot guarantee availability of classes and cannot reserve a seat in class until student is accepted.
Upon submission, your application will be delivered to our Admissions Department for processing and the $125 application fee becomes non-refundable. Additional information may also be requested in order to complete the application process.
